Chad Kodiak, R.Ph., Pharm.D. received his Doctor of Pharmacy degree from Purdue University, where he serves as Affiliate Clinical Professor in the Department of Pharmacy Practice. He is the second-generation owner of KODOCARE Pharmacy, which specializes in providing medications and pharmacy services to long term care facilities. KODOCARE Pharmacy is also a leading vaccine provider at both their on-site clinic, as well as offsite at various locations including facilities, businesses, and schools.
Kodiak has dedicated his career to preventative medicine through medication synchronization, compliance packaging, and immunizations. He created the Kodo Pak, a compliance package for improving medication management as well as other innovations to assist in the administration of medications. He is also a nationally recognized speaker and has presented at the National Community of Consultant Pharmacists national meeting, and at for numerous regional, state, and local organizations.
The University of St. Francis (USF), created the annual Sister Clare Award in 2013. This award is made to those who have made a positive, transformative impact and shaped the community; served as inspiration and role model and made a commitment to nature, the planet, and/or society. Kodiak lives in Joliet with his amazing wife and three wonderful children.
